Comprehensive Assessment
The assessment provides the foundation for our care management. Certified Geriatric Care Managers take the time to conduct an individualized assessment of the strengths and limitations of each senior. These assessments are then used to create the Solutions Plan.
Assessments generally include an examination of:
- Physical abilities and limitations
- Safety and appropriateness of living environment
- Cognitive and psychosocial status
- Family or other supports
- Current services or benefits utilized
- Financial or legal issues
- Personal preferences of the senior and family
